No, Malcolm McDowell's character doesn't die. You think he dies when the creatures attack him in his car, but at the end of the film, they have a "Six Months Later" where he's in a mental hospital and drawing the same disturbing images the little girl was drawing earlier. So the doctor becomes the patient after encountering the wild fairies.
