Why did his sister kill herself?
This part was a bit muddy for me.
What was the sisters relation to the kidnapped girl? The first time I watched I thought maybe she was the girl's nanny who the deaf man was conning into a relationship in order to get closer to the little girl.Then I rewatched that part and realized that was his sick sister, but she seemed very close to the girl. She's with the girl the first time we see her in the park as well.
And then seemingly she finds out that her brother A) lost his job and B) kidnapped that little girl? And then killed herself due to the stress of it all? What exactly about the phone exchange rattled her?

I have to say that all the exact relations to me are a bit fuzzy as well but I can answer your main question.

The sister quite often says she doesn't want to be a burden to her brother, so when she realizes he actually kidnapped someone to try to save her she kills herself. She doesn't want him to do horrible things to save her so she doesn't give him the chance to save her.
