I'm a big fan of almost all Miikes i've seen (15 so far), but this flick really got me from the first to the last minute.
Great story, acting, scoring and cinematography.
I consider it a masterpiece.

Absolutely true. This Movie is very underrated. Maybe because there aren't too many 'bizarre moments' in it. But it's a nice, moody picture about music, yakuzas and outsiders and it's a shame that it isn't as well-known as other Miike-movies that haven't got the quality that Blues Harp has.
At the Moment it's my favourite Miike movie as well, but I've just seen 13 so far. So there's still a good chance that I find another movie of his that I like better. But still I think, it will remain as one of his best.
It's my second favourite Miike, and you're right it is a masterpiece. Dead Or Alive 2: Birds is his best in my opinion.

Blues Harp is still my favourite and very rewatchable unlike Ichi and Audition which i both own but never watched twice.
DoA2 is great as well and a close runner up.
Gozu is great..
Graveyard of Honor grew on me...
