One thing

I caught this late at night on Showtime, not a bad movie. Give it a 6/10 or so.

Anyway, the Hero is supposed to be the Love Interest's first client, that's pretty important, some line about all the things that would have to happen for him to be her first.

But in the waiting room when the Hero first gets to her office, doesn't some elderly man walk around introducing himself as Dr. Something? Then the Love Interest comes out and says something like "Mr. Whatever, how many times do I have to tell you you're not a doctor." Obviously he wouldn't be her first client.

many offices like that have more than one therapist working there. it is possible that she encountered the man without him being her client.

It was actually Love Interest's secretary who ushered Dr. Hillcoat out of the office. She was in her office nervously preparing for her first client, as we see her when Blake enters.

It is a shared office. He and the other patients are waiting for a variety of therapists at the same location. It looked like there were at least three offices down the hallway past the receptionist.

A more important issue for the film is what happens after. By pursuing a relationship with her first patient she is almost certainly ending her career. Even if she officially discontinued treating him before they go out together, her now jilted fiance can cause serious trouble just by reporting her behavior. If she had a solid reputation, and an existing career, she might whether a review; but falling for her first client is probably cause to lose her license.
