A Couple of Scenes That I'm Confused By

I really think I'm missing something with the scene in the hospital where Sarah is putting blame on Johnny for what happened to Frankie. It seems a little absurd that she'd do that and why is she doing that in that moment??? Is it just because her emotions are high from the birth I suppose? And to put blame on him for putting the gate up doesn't make sense because the gate is meant to protect. My understanding is it was just a freak thing that happened to Frankie and no one is to blame.

The other question I have is... in an early scene Johnny is mad at Mateo for apparently trying to persuade Sarah to let nature run its course with the baby and not intervene and I'm guessing induce or c-section the baby early for her safety. I don't remember a scene in which Mateo does this... was there a scene where he does that or is it just referred to this one time?
