That's one of the great ironies of the movie. They left Ireland for America but in Ireland she would have received free medical care from antenatal, through delivery, to postnatal care. And it's not just a free clinic type thing, any GP and obstetrician can be used. Obviously they don't get paid as much by the government as they would by an insurance company but they're not allowed to refuse. It's against the law.

I've had a lot of sobering thoughts in my time Del Boy, it's them that started me drinking!
