that paladine transfered his soul into the girl's body and then killed the old lady who I assume had heather's soul in her.
sharePalladine was the old lady Doctor. She identified herself to Gorn's character when he comes to tied to the table in the apartment. He's been transferring into bodies for five centuries according to the script.

What happens as far as I could tell was that since Heather was hypnotized into fearing nothing(2nd time hypnotist made this mistake!-just stupid I guess)the girl did not fear transference of soul process wherein she becomes ancient villain. After that she's ordered to kill remains of old lady which she also does not fear. Only logical explanation I could see.

*the method of transference is left purposely vague to give the momentary impression that good triumphed over evil before the clumsy twist ending shows otherwise. If you missed the needlessly overt shots of the finger twiddling earlier in the film, it's likely because you fell asleep.
