Doctor Orloff!

In the scene where they're all in the car laughing about how Martin tried to kill himself Orloff's girlfriend laughs when Martin says "Dr. Orloff" as if he wasn't a doctor. I thought it might turn out Orloff wasn't even a doctor which would be funny as hell. The hospital referred Martin to Dr. Orloff, so I don't see how he couldn't be a doctor. Maybe I'm reading into it too much but I really think something was implied in the scene.
