I've known children who call their parents by their first names, though it is rare, and was especially rare back in the day. I know a lot of guys who are referred to by their last name, too - I've even had a few male teachers call me by my last name. It's kind of a guy thing, sometimes.
As for Miles referring to himself in the third person and saying "And then Miles got married..." - well, that's open to interpretation. I always sort of thought that he got so immersed in telling her the story that he distanced himself from it, and also that he wanted to distance himself from the past because it hurt too much. It seemed to me that when he was telling the story he had this faraway look in his eyes, like he was watching the story unfold in his mind's eye, just as we were watching flashbacks. So he was describing the scene, like "Miles got married" like he's a character in the story instead of himself, telling the story.
I don't know if that makes sense, but that's how I took it.
