when Johnny and Sarah were at the carnival, did Johnny already know that Sarah was pregnant which leads up to the part where she asked him how many kids he wanted or was it one of those things that could have happened off screen and we had to put 2 and 2 together?

I don't think he had any idea that she was going to get pregnant that night, but it's implied that she did. It faded away from them in the boat, then he dropped her off and got in the wreck on the way home. Sadly, too many shows now show everyone having sex and leave nothing to the imagination or to be deduced from the context. I appreciate shows like this that are of the old school.
