Mysterious Skin is pretty unknown and dealing with similar issue as MBT. I have both on disc, but after watching MBT, I couldn't watch another so depressing movie on same theme.
If you like nature, maybe you would like (the best) slovak movie Zahrada (The Garden, 1995), it's available as dvdrip with eng subtitles on net.
Other good european movies (maybe unknown to somebody), most of them depressing: Lilja 4-ever 4 Months 3 Weeks And 2 Days The Bothersome Man Italiensk For Begyndere The King Is Alive One Point O
And some good, but not famous US movies: Ghost World Igby Goes Down Me And You And Everyone We Know My Life Without Me The Good Girl The Woodsman

Yeah I've seen most all of the US ones (all but My Life Without Me). Mysterious skin is really really good, but i wont be watching it again for a pretty long time. Lilja 4ever is really desolate, but I agree worth watching. I'm going to have to check out the others.
Other films that I liked that are ultimately less depressing are the Chummscrubber, The Thumbsucker and they arent rated so highly, I also like Stay. Lightning Bug is pretty good if you can find it (but it goes a little crazy towards the end)
The french film Delecatessen is only rated around 7 on here, i think that it is better than that.
i think the 2001 Irish film Disco Pigs is quite similar to My Brother Tom and they're both underrated here on IMDB in my opinion. i gave them both an 8, but their average IMDB user ratings are in the 6s.
I've never considered the similarity between disco pigs and MBT but I definitely see it now. It was exactly what I was looking for after watching MBT, so anyone who's reading this and is on the fence should definitely give it a go. Both films have such beautiful talented acting
