Please help me with locked multiplayer levels

No matter what I do, 5 of the multiplayer levels remain locked. I've beaten the game, tried cheats etc. Nothing seems to work. What's the secret to unlocking all the multi levels? Please help!

I think certain levels can only be played with specific teams, or game types.
