O'Toole's arm (update interview)

On the dvd there are some follow up interviews. There is one with O'Toole and you can see on his right forearm there are chunks of flesh missing that have healed over. It is all skin colored but if you watch when he gestures with his hand, the shape of the muscles are moving. Anybody know what's up with that? What happened?
