Does the book explain how Carol died ? Anyone now ?

In the book Carol does not die. If I remember correctly she faked her death and changed her name. She said something like "I don't know anybody named Carol. My name is Denise" and then they started talking about how Ted fixed her arm when they were little and stuff.I hope I'm not getting confused, it has been about a year since I read the book.

I could never quite get through this book either. I also struggled with Duma Key and Insomnia.
