Interesting conundrum you created. So for a heart transplant that costs a quarter of a million dollars, a gun should be held to someone's head to perform the surgery? Who's gonna pay for the labor and the heart and all that?
You advocate another form of 'hostage taking' which is no better than the one you condemn.
I don't advocate either one, actually, I was using it as a comparison to point out the similarities between the two forms of extortion.

Ethically, those two situations represent two entirely different scenarios. In the first one, an individual is initiating aggression by THREATENING someone's life for money. In the second hypothetical, someone is simply demanding compensation for a service. That hardly constitutes extortion.
As someone above mentioned, you need food to survive. Do you expect to get that for free as well?
Difference is if you can't afford food you can get foodstamps or go to a food bank or soup kitchen and get food or be feed you can't do that with health care now can you?

I can think of a hundred situations where a good amount of money can save someone´s life. The question is: from where this money will come?
