Hey. Here are my pick for the director's whose films make people squirm.

Todd Solondz
Harmony Korine

David Lynch
John Waters(the early films)
Peter Greenaway
Larry Clark
Ken Russell
Lars Von Trier

There are many more and a good deal of directors who've only directed a few films or even just one deserve a mention here.

I've seen at least one movie by everyone there except Ken Russell. However, I do believe you forgot Park Chan-Wook, one of my favorite directors of all time. Watch Sympathy For Mr. Vengeance, please, as well as the rest of the Vengeance trilogy. That is one of the few movies that almost made me feel physical pain watching it, yet still was a brilliant character study and work of cinematography.

I think one could argue for Eli Roth's inclusion on this list...but hey...I think it still looks pretty good.

Would not really say Harmony, but to each his own.
But one that needs to be up there. Is the fantastic...
Gasper Noe. or Gaspar Noé.
Also Mr. Dario Argento.

In my opinion Greg Akari (or Araki?) is a pretty twisted director too: The Doom Generation, Nowhere, Mysterious Skin. His next movie is some sort of stoner movie with Anna Faris. I guess maybe he is going for a lighter touch on that one...maybe.
