Ive seen a few crazy movies in my time, this being one of them. For whatever reason, whether it be sexual, pshycological or over the top violence, these movies really are the ones that take things too far and and have left a lasting impression with me.
For all those people that liked to be shocked when watching a movie, here are some suggestions for you..

I only felt two(thus a couple)films on my list were garbage, but they definetly qualify as messed up movies. Salo and Cannibal Holocaust were just uninteresting, but shocking non the less. I am an English major and will check out the deSade's version of Salo. Really want to see how much worse he couldn've made it.

Salo is the name of the film somewhat based on De Sade's "The 120 Days of Sodom". It isn't a book I recommend lightly, by any means. I recommend an iron stomach and objective moral compass.

"Worse"? The film is but 1\1000th of the book's depravity. Considering De Sade founded "Sadism" I would go so far as to say that "The 120 Days of Sodom" is the sadist's bible in every sense of the word. The "source" if you will of the modern understandings of extreme sexual perversion, sexual torture and the true natures of man.

I never really understand why "Dead Alive" is considered particularly disturbing. It is ultra violent, but it's the type that's so overblown, it makes for a f'ing hilarious film.
Silence Of The Lambs / Hannibal - the Lecter character is a profoundly disturbing portrait in its complexity
Dead Man's Shoes - so depressing, so senseless and unlike some vigilante films, the violence doesn't seem cathartic
From Dusk Til Dawn / Kill Bill / Death Proof - Tarantino's 'by the way' approach to rape and torture (and implication of titillation) is always disturbing to me.
Linked to the above point we can list some of the video nasties and exploitation films which QT grew up on such as 'Last House On The Left' and 'I Spit On Your Grave'. I love QT but I do find him disturbing.
The Vanishing (1988)(original Dutch title Spoorloos) - spoiler alert - it's all about the ending. Was fortunate enough to see it with only a vague inkling of the end.
The Wicker Man (1973) - spoilers. Same again. And same again only had an inkling.
Bad Education (La Mala Educacion) - a rare complex portrait of one of the ultimate tabboos - paedophilia - and therefore incredibly disturbing.
Ring(or Ringu)(1998) - again, if you're not expecting the big scare, it is effective. Masterful build-up. Watched it sat right in front of the telly so I didn't wake the folks too - so that added to the effect!!
Irreversible - obviously. Though I commend its power and its challenge to the audience I'm not sure I think the world is a better place for its existence.
Michael Haneke's films are also quite disturbing though somehow it seems less about titillation than psychological / sociological analysis. The scene on the Tube train in Code Inconnu (Code Unknown), for instance - truly powerful.
Been a while since I saw Bully but I don't find Kids and Ken Park especially flucked up. It is extremely rare to see youth depicted honestly which is what Larry Clark is a master at doing.

Of the films listed that I've seen, "A Serbian Film", "Irreversible", and "Martyrs" are, to me, the most disturbing. I cannot get the second to last scene from "Serbian" out of my head. People make a big fuss over the baby, but the sheer horror in the main character when he realizes what happened in that second to last major scene made me sick/depressed. Very effective.
