A whole city/town (?) poisoned by contaminated water... Where was this place?

There's "Breaking The Mold," the episode about the Ballard family whose house was so filled with mold that it made them seriously ill.

There was also an episode about a computer hack attack. The programmer was fired and he got revenge by hacking the company's computer system. It wasn't violent but it was terrible.

There's an episode called Deadly Formula where a baby dies and they think the mom poisoned him because there was polyethylene glycol in his system but it actually turns out that the baby had a disease called MMA (methylmalonic acidemia) that somehow caused the toxins to be in his body.
