As I remember (and it's a while since I've seen it) the ending in fact implies that the protagonist has become obsessed with spirals herself, hence how she begins to tell the story again from the beginning. It's circular you see. Quite a depressing ending. I doubt there will be a sequel.

The ending, disappointed me too. I loved this film, and I love the manga. The reason it ended like that was probably because of the budget. It was a very low budget film, hence why they probably didn't have any of the deaths or extra off events in motion. I doubt they could've afforded, say, the part where the 'medusa' character's hair wraps around the pole and her skin starts pulling off. The part about the character telling the story again works for me though.

Oh my god! You killed Fritz! You yellow stinking fairies killed Fritz!
