It seems he did choose to kill himself. Since the accident happens off camera, and we last see him trying to face his personal situation but not seeming to find happiness in either scenario(staying with his wife or leaving her for his gay lover ) it stands to reason that he himself causes said accident in order to escape having to deal with the outcome of facing up to his dual life. I agree, that it would have been nice to see him find some sort of middle ground, but real life(unlike in televisionland) does not always have a happy or desirable conclusion.
I agree that the opening is wonderful, the sex scenes are realistic and graphic-with two decidedly straight actors-and the story seems well done, though I too had mixed feelings about the ending. This film came up in discussion while comparing the recent Brokeback Mountain to other gay themed films involving essentially a bi-focused storyline. Frankly I thought Segunda Piel was a much better movie and proof once again that the spanish(and europeans) have a much more effective grasp of sexuality in their cinema than the general american cinema does-though there are certainly exceptions, Greg Araki's recent Mysterious Skin, for example.
Hopefully people will seek out this film. it is certainly worth seeing.
