Fairy/Nurse - did I miss something

When Joey drops off Nikki's kids at the club he is mesmerised by the child minder dressed up as a fairy. It is the same actress who is the nurse in the last scene who winks at Nikki. I get that the last scene is not reality but Joey's fantasy of Nikki giving a press conference and saying she loves him (I assume Joey either killed Nikki or badly wounded her in the previous scenes). So who/what does the fairy/nurse represent? Is the same woman who was the self-help presenter in the first scene?

I didn't get it at all, but both times it seemed signficant.
