I think Kain's transformation reflects his nature within. Actualy, there are good exanples of character whose nature within surfaces in a transformation.
Voldemort, for example. He was once Tom Marvolo Riddle, not only a student of the Slytherin House, but also heir of Salazar Slytherin himself. Today, as Voldemort, his face resembles a snake.
Another example are the cases of Clan Gangrel and Clan Nosferaru, from Vampire books and games.
And, if you consider Kain's Lieutenants:
RAZIEL: He is the ambiguous champion foretold by the Vampires' prophesies, as both Redeemer and Destroyer of Nosgoth. As a figure of the Ancient's beleief, he is expected to grow wings. When Kain saw his first-born child with his evolved wings, he bet Raziel was ready to be put to the tests to fulfill his destiny.
TUREL: There is no backstory to explain Turel's evolution, but, seeing how he is in Defiance, we can make a paralel between him and the Elder God: both are horrible, and both are tought to be gods - the earlier is the Oracle of the Ancients, the latter is Hash'ak'gik himself.
DUMAH: We all know Dumah was always arrogant and considered himself invencible. The invasion he and his brood suffered between Raziel's execution and ressurection is a proof of it. So his vanity was such he evolved into an armored titan.
RAHAB: He had two basic weaknessess: sunlight and water. To flee from the earlier, he surpassed the latter, so, as the centuries passed, he became a amphibioua-like monster.
ZEPHON: Like Turel'a, his evolution is not well-explained.
MELCHIAH: We all know Melchiah is the "something rotten in the kingdom of Denmark". Since his birth, he invaded the cemetery near his Necropolis to pick up the carrions to cover his decaying flesh. So, he became a blob of rotten flesh (thank god we cant smell the things inside the games!).
