Confused

In the scene where Zinaida's mother was asking the Vladimir to write a letter for her, an old woman walked across the room, and then Zinaida's mother went "Zinaida! Zinaida!" and then Vladimir goes "she know I was in love" (close up on Zanaida looking through the door) "and that I was no the only one"

What was going on there? I didn't understand that scene at all, can anyone clear it up for me?

i think that dunst was not available at that time for the scene so they found an old lady to replace her that scene.
