From The Big Picture Show Ed asks "Does it hurt Double D?" giving the implication that Edd probably has something on his head that is both disturbing, funny, and probably painful. He's likely bald though with just those few hairs on the back of his head.

On cartoon networks website there's a Ed Edd n Eddy game ( I think it's the food fight one) if you play as double D and win he removes his hat on celebration. Under his hat are a few strands of hair. There's also a video on YouTube if you don't want to play the game

I always imagined it was a giant brain or something. It explains his intelligence and why Ed and Eddy seemed so frightened or weirded out whenever he would take off his hat.

I actually remember seeing it in one of the episodes and it looked like cornrows going back. That's what I seriously remember. idk what episode but I def remember seeing it a long time ago.
