... at the end, Kiefer Sutherland's character REVEALING himself to the semi-conscious Farrell in the ambulance that it was him, how was he SO sure he wouldn't later say ANYTHING about this to the police, and why didn't anyone including tons of police officers at the end notice this and how was he also able to walk right past the crime scene with a rifle case without anyone batting an eye lid?

Or was that just Farrell's hallucination?

Well, it's just your typical twist ending of the bad guy being alive. I'm sure the Caller wouldn't worry too much, since Stu's vision is blurry. What I want to know why nobody was watching Stu after being the victim of a crime and sedated.

1. They already "caught" the killer
2. It wasn't a rifle case (looked more like a workers case)
3. Farrell was about to be driven away. There was a small moment of opportunity where Kiefer could walk up to him and have a chat. Also, by keeping Farrell in conversation it looked like someone was actually keeping him company, i.e. safe.

It was just so Kiefer could have some screen time. And the protag could meat the antag IRL.
