Can Snake actually see who he's speaking too via Codec. I always wondered, because when Snake is first introduced to Mei Ling he says he never expected someone who does her job to look so cute.

There's a few references like those that point to yes. But I've always been interested in what the codec actually looks like in the game. We never see it. Snake just touches his ear and looks to the ground. Excluding the Snake Eater radio.

in the beginning Campbell explains that the codec is on the small bones of snakes ears so no he cant see whos talkin to him only we can see

I think that Snake being able to call Mei Ling a cutie is supposed to be an inside joke between us (the audience) and the characters of the game. It is meant to help gamers feel as though they ate even more apart of the game than they are.
