What happened in the end? (Spoilers)

Sonny ends up getting shot and dies, becoming part of the town folk in truth. Blackjack suffers the same fate.

The Indian shows up to collect the bodies of Blackjack and his men. He takes them through the gate and over the hill, and we find that it opens up on to a giant, deep pit with a lake of lava at the bottom (Hell). The Indian tosses the corpses into the pit, and we see that Blackjack re-awakens as he falls, screaming just before he falls into the Lake of Fire forever.

Meanwhile, Doc Holladay and his allies are standing with the Indian at the gate, about to go in, when the Coachman shows up. He says the Almighty is "tough but not blind", and says that it's time for them to move on to Heaven. He makes the same offer to Sonny, who chooses to stay behind a little longer so he can be with Rose.

Doc Holladay and company board the Coach, which takes off up a hill. As it climbs higher, bright white light surrounds the coach, and heavenly singing is heard. Doc Holladay looks outside through the curtains and smiles. The End.
