Since you never got a real answer on your question, I thought I'd let you know that at the end, Jane takes the boy to the lake she used to stay at when she was a child. During the car ride to the lake, Joseph Gordon Levitt's character is in severe pain as he's dying. But, they finally make it to the lake and they go out in a rowboat that's on the edge of the lake. While they're out there, the boy begins to have fits of agonizing pain, so Jane pulls out the balloon of herion that she bought earlier in the film and she ends up injecting it so the boy won't suffer. It fades out with a faraway shot of the boat drifting on the lake and Jane holding the boy and sobbing.
