I think, maybe, he was standing in front of the toilet, which probably DIDN'T have a mirror on the wall behind it. When he was through and trying to find it, the doorbell rang and he got in a hurry. He just didn't see it in any place it should have been. I remember he even looked up, which made me laugh out loud. My guess is, he just wasn't expecting it to end up where it did and he was never standing directly in front of the mirror by the sink.
