I know Blaster and Soundwave died. (Transformers: Headmasters - Soundwave and Blaster duel, youtube)
Then Soundwave was rebuilt and renamed Soundblaster.
**Spoiler Alert**
In a quick summary of deaths disspareances and rebuilds:-
Blaster was rebuilt as "Twincast"
Kup, Rodimus, Blur and WrekGarr vanish partway through the season on a mission to search for a new world.
Optimus Prime dies permanently and does not get resurrected in Masterforce, Victory or Zone which followed.
Ultra Magnus dies at the hands of six shot.
Scorpnorok/Zarak techically do not die as only his body, the lifeless Trastector gets blown up and not the head.
Interesting additions to this series in my opinion think are Sixshot, and the Trainbots and thier Combiner Raiden. The somewhat arrogant Target Masters are also interesting!

The Trainbots are an interesting addition.
I'd like to see what planet Rodimus and the rest found.
Transformers needs to be started again from the point of Rodimus and the others go on their mission for the new planet.
Another strange thing about Headmasters is that Kup never transforms once!
