It's dedicated to her at the end of the film, indicating her date of birth and death, but I can't find any information on whether this film was based on a true story.

http://www.anglicanhighway.com/mreviews/movieT04.htm this article mentions that there was a documentary, but that too I can't seem to find. Anyone know?

No, at the very end of the credits it says all of the characters are fictional
