This is a kinda odd question, but is there such a thing as fake heroin, or is it just called something else? Perhaps medicinal? And why the hell would you mix it with milk?

Heroin is already what's called a "semi-synthetic" if naturally produced morphine (as extracted from opium generally) is used as the precursor. Mind you, in this day and age this is no longer generally the case, just like few chemists use ergot to make LSD anymore (with more efficient and effective precursors being around than ergot), most heroin manufacturers use already synthetically produced drugs as precursors and so, in truth, most heroin out there is already "synthetic".
