'Did it hurt?' etc. with Illeana Douglas

I may have missed something, because I haven't seen the movie in a while and recently tuned in halfway through, but what is Illeana Douglas talking about when she says "it didn't hurt" or answers other people's questions about whether "it" hurt or not? What are the people asking about? Did what hurt? Are they referring to something about her nose? (After getting asked once, she says her nose bled profusely, but I don't remember anything happening to her nose, and she sounds as though she's being totally sarcastic.)

Just confused and wondering.
