Did Billy Bob use prosthetic makeup for the role of Karl, or was he just sticking out his chin?

He looks NOTHING at all like the standard Billy-Bob we have come to know since then in other films. I would never have recognized him if I had just seen a clip that had not identification of him in it. I'm trying to figure out if he wore special effects makeup or just achieved the look by jutting his lower jaw.
