Definitely Shunji Iwai's best short film, and to my mind, infinitely more coherent than Yentown.
Everything works for the narrative. The script is tight, the acting's suitably restrained, and the walltop marathon is an absurdist road movie to compare with the Milky Way.
I'm sure there's a ton of allegorical baggage in the subtext.
I suppose that what is generally regarded as mainstream sanity is a bit of a balancing act. Sometimes people fall.
Having spent a portion of my life in the company of schizophrenics, I found the portrayal of the three leads to be nicely restrained. It's easy to caricature mental illness, but it's much harder to portray it accurately and sympathetically.
