The ending is that Alan goes to the school where he teaches dental students, on the run from the police. He hears an opera singer and when he gets her to turn around, he hallicinates that it's his wife. He drops to his knees and the police hold a gun to him. He imagines that Brooke is laughing at him (she is wearing her black dress she wore before he tortured her). It shows him in the mental hospital where the guards take him out for his "regular visit" which he seems frightened by. They push him into another room with a straight jacket on where there is a dental chair and his wife is the dentist. It has a hallucination effect where she holds open his jaw and smiles and you can see she has no teeth and then she starts drilling him. The camera zooms out slowly until it fades to black.

Now for the tortures - with Brooke, he tricks her into coming to his office and coaxs her into a dental chair, in an opera themed room for her "anniversay teeth cleaning". When she sits down, he ties her neck to the chair while she struggles and he gases her in order to make her stop. He ties her arms to the chair and takes off his jacket. He picks up a thing which holds open her jaw and then gets dental pliers and pulls out one of her teeth. As it is bleeding, he says that she will not get any mercy tonight. He then proceeds to pull out her other teeth before it fades to black with her screaming. It is a built out scene.

He kills Matt by stabbing him.

He stabs the needle which has a numbing agent, hard into Paula Roberts' gum and uses the drill on her molar and strangles Jessica with pantyhose after she threatens to tell the dental association about him molesting April Reign.

He tortures the IRS agent by breaking his jaw and drilling his tongue.

He kills Karen by injecting an air-filled syringe into her jugular vein.
