I would say that collapsing is a pretty good sign of a miscarriage. She didn't just pass out and then say, "Hey, I think I'll get an abortion while I'm here."
Besides that, your standard clinical abortion doesn't involve profuse bleeding.

PLUS It was too quick for sex and even though he made jokes about it, it was more than likely a defense so they wouldn't know he was telling her a way out. Everyone already thought she was a whore so why not play into it?
I don't think she "collapsed" - she was throwing up - a sign of morning sickness. I agree that it was an abortion - mainly b/c of the nurse's obvious contempt for her and the comment that she was "the 5th girl this week."
All the comments made by the nurses... indicate they thought she was just paying the price for her whore-ish behavior.
And, I'm sorry, but that was anything BUT a "standard clinical abortion" - it was barbaric and, IMO, another indication of the nurses' blatant disregard for her condition. If it were a "standard" miscarriage, it wouldn't be so bloody either and I think the nurses would've been more sympathetic with her situation...

2) The two toe kid does not rape her nor does she have sex with him. Remember that in a previous scene, he and his friends are talking about the "poor girls" inability to get out. So the two toe kid goes to xiu xiu and gives her an idea to shoot her own foot. That's what he meant by "intellectual discussion".
3) After all that has happened to Xiu xiu in the camp, she does not want to return to chengdu because her simple parents cannot handle the shame. When she says she wants to leave, she means "suicide".
