At the start we see Tennison go to hospital to terminate the pregnancy she conceived during the previous series however The Lost Child aired 18 months after Prime Suspect 3.

In my opinion, it looks like a flashback, since it opens with Tennison going to work.

It is considered a goof because the person was unable to distinguish between real life (18 months between episodes) and fiction (the next episode is the next part of the fictional time line).
