I was hoping to find this very fact in the goofs section but its not listed.
I am very sure train must have toilets.The entire thing was ignored in order to forward the plot.
Now guess what ,moron Indian Bollywood copycats made a remake of the movie and have the same no toilet goof. All Indian trains have toilet as well.
Im sure there was, Kate was so sick though that she needed a bathroom even after they got off the train, that she wasn't over her sickness from all the dairy!

It's not that there wasn't a bathroom on the train- she was getting sick from the motion of the train was well, she needed to get off to get away from the motion.
