what DD really is about *spoilers*
I think everything takes place in-head. (i even think it's obvious)
Heres my brief try of an explanation:
There is a man named Franco. He used to be an engineer. His family-matters didn't go well, so he cheated on his wife ('the first love doesen't count, its the last love that counts'). The mistress was a whore, which he fell in love with. But she didn't fall in love with him (of course... she's a hooker). He could't comprehend and killed ('Death, death, death the whore')the slut out of jealousy ('I shot her with my eyes closed'). After the murder he got insane and went out on a killing spree (7 pedestrians and his family).
So far ... so imaginable.
He is now in a hospital suffering a irreversible near death coma (maybe even this ain't real) and his mind tries to work out his guilt. (this is the theory!)
This is where the movie starts!
Francesco (notice the namesimilarity to Franco) represents his dark side, the murderer.
Gnaghi symbolizes the supressed remains (he is not even able to talk right) of his good side.
The zombies are Francos inner demons (feelings of guilt) which haunt him and which he tries to fight off.
Lei (ital.: She) represents the forbidden fruit. (She also is his worst 'returner'. 'I don't want her to come back')
In the end, when Francesco and Gnaghi have passed the tunnel (a common symbol for near-death experiences) the good side (Gnaghi can talk, Francesco murmurs) takes over and the engineer can rest in peace or whatever.
proof for my (in-head)theory...
-all characters are acting if they're one beer short of a sixpack (e.g the vomiting)
-why would a handsome guy like Francesco have no friends. Franco is the only one who calls him sometimes. (there is no one else)
-why in the world would anyone call a cemetery man an engineer? It doesn't make sense at all (unless he is one in real life).
-the zombies are never called zombies but 'returners'. (because they are no real zombies but his inner demons which constantly return to haunt him)
-why does the policeman never suspect him despite it is so obvious? (weird, ain't it?)
-everytime he's on the phone with Franco, returners appear (his guilt surfaces).
-the 2nd 'SHE' answers his question 'do you love me too?' with 'why? who else is there?' (sure... the stage in his head, he is the only one 'who's there')
-he wants to get rid of his wiener because he recognizes that it was his doom!
-Franco (the engineer) refuses to know his supposed only friend (denies the existence of his dark side)
-Franco's hospital room has no doors and no walls, lies in the dark (representing the capsule he is in)
-Franco wants to die ('everything is sh.it. the only thing that is not sh.itty is sleep)
-Grim Reaper:'if you don't want the dead to return, kill the living. shoot'em in the head.' (the meaning is not 'headshot', but 'kill them in your head')
-Grim Reaper: 'where do you want to go, when you don't even know the difference between life and me' (you have to work up what you did, you can't just leave. it won't go away.)
-as Franco tells him to 'GO AWAY', Francesco immediately leaves town.
-Francesco, being a prisoner of Franco's imagination, never got out of Buffalore and claims to know every streetbump! When he first gets out of Buffalore he recognizes that the streets are wider than all the streets of Buffalore put toghether. (Buffalore = Franco's mind. Emphasized by the encapsulation in a snowball)
I hope that's enough proof for now.
Watch the movie again using my theory and let me know what you think.

btw: imo it would have been better with less gore (watched the OV) it doesn't need it.
