He indeed fell of the floor. You'll see this in the first minutes of the movie. Shep and Nutso were playing basketball on a roof.
shareShep's high-school teammate who was killed. They show him falling from a building to his death at a young age. It appears as a basketball accident, but may have been deeper than that. He amy have been thrown off a building and Shep had been hiding it. The accident doesn't seem too realistic anyway.
