Do you mean how it was filmed? I imagined the filmmakers had filmed Anna Paquin peeking into the door and then placed a shot of Ada and Baines having sex, blocked out to make it look like a little hole in the door. I doubt the actor herself was allowed to see what was actually going on.
As for the character, yeah, Flora sees them and understands that her mother isn't playing piano all the time at George's.
I'm sure Anna Paquin did not see what we saw with Baines and Ada. Flora didn't like seeing her mother give her attention to that man so she dropped a hint or two. "How are the lessons going?" "Mr. Baines doesn't play. And sometimes she doesn't play at all."
