Check out Six Feet Unders! It's a a tv show, but Jeremy Sisto is one of the main characters (kinda) and he plays a bipolar character. He is great and does a very realistic and touching portrayal as a bipolar person with some psychotic touches.

I can't remember if it's manic-depressive behavior or bi-polar but Kevin Kline in "Sophie's Choice".
