It isn't that to enjoy this kind of movie you need a certain level of sophistication, intelligence, or education, it's that you simply need to like this kind of movie. Whether you enjoy a movie or not is halfway based on your own perceptions and feelings about it, and halfway based on how well the movie was made and the extent to which it can be enjoyed by the average individual. If you don't like it, then fine. Maybe you don't like character studies, or movies that take a while to get you involved, or movies that have a slower pace, or movies that have a thought-provoking air about them, or any of hundreds of other characteristics. And the ones I just listed were based on my own perceptions of the film, someone else may see it as a movie of soulmates working through lifetimes until they get it right, who knows. But whether you believe a movie is good or not is basically almost entirely based on how you see it.
Just for my own two cents, I loved it. I was originally watching it to see Ewan McGregor, and then saw Vincent D'Onofrio, John Tuturro, and even Robert Carlyle as well. What great surprises! I love the idea for the movie, and really enjoy movies like this. Different sort of stories, maybe those that make you think about other things differently, or at all. Most recently, 'Happy Accidents' did that for me as well. Another awesome movie.. But that's for another place..

Edit: Wait, one more thing. I think someone else already addressed this, but the fact that most people haven't even heard of it doesn't really speak to how good the movie was. There are other, spectacular movies that take a very long time to become well known. Some never get to be well known, but that doesn't change the quality of the film itself. Some of the best movies can actually seem better when only a few people know about them.
