A Stepfather IV is possible but this time he is going to need some SERIOUS plastic surgery!
Actually, many sequels had been made that were based on the same ideas of the original films without actually continuing with exactly the same characters - including for example, series like Final Destination, Wrong Turn, The Hills Have Eyes, Single White Female, I Know What You Did Last Summer, and many others. Actually, it would be better if Stepfather III had introduced a new character rather than using the ridiculous idea of plastic surgery. The character looked like ten years younger. Did the plastic surgeon use some youth-rejuvenating drugs too?
It would be pretty easy. All they would need to do is say the stepfather from part 3 was a copycat. He got his face switched so he could look somewhat like the stepfather instead of the other way around.

Films are not reality. Reality is not film. Film is only an approximation of reality.
That would actually make more sense and be a nice opportunity to re-introduce Terry O' Quinn back into the series. Part 4, 20 years after the events of part 2 :)
shareOr say that he ended up impregnating one of his earlier surviving wives, or perhaps impregnated a girl from his teenage years and that son inherited the psychological gene, the "killing gene" and slowly but surely becomes more critical of his family. Then things really go awry when his wife is cheating on him. Then finds out his daughter is loose, then it just all goes straight to hell.
Think about it. He can appear normal at first, he takes medication and sees a counselor, then he slowly stops taking meds against his doctor's wishes and then that's when those previous incidents occur and it goes straight to hell. After that, he becomes a full fledged killer. Also during the movie, he will somehow piece together that he is the bastard child of the stepfather.
Prequel? *cringe*
