So... The bad guys take over a hospital and start fighting with the cops, but why? The film doesn't explain why they did this and there is no reason or motivation explainable. Am I the only one bothered by this?

well, besides the fact Tequila & Tony planned a raid that night, the bad guys' (Johnny and crew) weapons cache is hidden under the hospital, remember? i assume they had the hospital officials paid off.. or maybe scared.
The reason why the bad guys took over a hospital is never explained, although my take on it was that a hospital was chosen just to show how ruthless Johnny was and no other reason besides that.

The whole point of the movie is that the undercover cop is trying to locate the weapons cache of the main bad guy.
Well, turns out the bad guy's weapons cache is hidden in a secret added-on basement to the hospital. It's accessible through the false morgue cabinets.
When the good guys discover this, they naturally break into the weapons cache. The big boss is none too pleased with this though, and so he begins murdering the patients 1. as leverage in the situation to show how dangerous he is, and 2. because he's honestly just a bit whacko.
It's over the top, unrealistic, maybe a bit silly, but it DOES make sense and it is clear what's happening if you're paying attention.
