Are the first two islands they arrive at real and real small or fictional?

Those islands were totally fictional. It was filmed in Puerto Rico the whole time, in several different places. When they were in Cuba it was actually a set in PR, altough the name Aguadilla exists in the northwestern part of the island, clearly isn't Cuba. At the end, when the Captain disappears it was filmed in one of the Cruise Piers, and it had a signs saying it was Miami, FL. I noticed an error in the end... They were in "MIAMI", however you can see some buildings of San Juan in the back of the screen at the other side of the marina, I even noticed one of the boats with a PR (Numbers)boat license. Then I think they used another marina in the northeasternn tip of the island, Fajardo, you can see the mountain peak next to the beach.

According to the screenwriter's blog, "The two islands in the beginning of Captain Ron represent Heaven and Earth. My idea for the movie was based on the book of Genesis. The director completely messed everyhthing up by casting a bunch of rubbery faced, washed-up Hollywood hacks and an unworthy crew."

I tried to contact him for further information, but he just sent me a rude picture of himself. Oh well.
