"The only good copper" - writing blooper

I've been watching the re-runs on Drama channel, and discovered a real blooper in this episode.

At the funeral, the wife of the dead pc says to Tony "Not Tony Clark of complaints? He couldn't sleep with worry, it made him careless.... you killed him"

She couldn't have known his name. Even if the investigation had been started by the department before his death, Tony hadn't been involved in any way. He was brought in after the death of her husband.

A generic "You lot killed him" - I could buy. But Tony knew nothing about the guy when he was brought in, so he clearly hadn't met the wife or even crossed paths with her husband before.

Major error there.
