Was Alexander Delia's brother or son? How was she pregnant with her own brother? I don't get that.

shareA twin can carry their own twin. It's being shown on plenty of medical shows. Many twins with extra body parts often in odd places is the result of the partial twin being absorbed.

Its called fetus papyraceus. But what's a bit odd is that usually the twin dies in utero while the other lives. But I don't know everything there is to know about it, so I can't say.
Perhaps the evil doctor in the film knew this, removed it before it died, and allowed it to live by implanting it in Karen.

Delia had a twin brother who never developed, his fetus was absorbed into Delia while they were in the womb. Somehow the doctor was able to remove the fetus from Delia and implant it in Karen and have it grow into a new baby. I doubt this is medically accurate but then we are talking Satan here.
It is interesting though that they had her start her period at 8 like that was connected to her carrying the embryo of her twin. I so wish they would have shown more of the background regarding this plus the doctor's involvement and the nuns involvement regarding Delia.

One thing I didn't get was how did the doctor remove the baby from Delia, and implant it in Karen without Karen's knowledge?
I would assume scalpels are necessary, right? Wouldn't the parents notice the cuts on Delia (not from a fall off a horse)...and Karen would know she was cut.

Well actually it was an egg not yet a baby that was inside of Delia. I am assuming he would have been able to remove this non surgically from Delia like with a needle or something. He secretly did this when Delia was brought in after she fell off the horse. The doctor implanted the egg into Karen when Karen came into the hospital after she kind of freaked out about the nanny Joe getting killed. At one point remember Karen was sedated with something to make her relax. She was put out and presumably the egg was implanted into her. Later on when the father was waiting in the hospital waiting room the doctor lets him know that Karen was pregnant.
