Terrible scene framing

Howdy folks, btief question
While watching the film, I noticed that a lot of scenes have character's foreheads cut off. I double checked, and the aspect ratio on the DVD is 1.78:1 for 16:9 sets. Is that a choice made by the director/editor, or is it me not setting the aspect ratio properly?

P.S. The slightly aggressive title was more or less a way to get your attention. I am not saying that the film sucks because of this, I am just wondering

Decided to rephrase my question a bit.

The sentence following the aspect ratio specs refers back to wondering if the director/editor chose intentionally (or unintentionally for that matter) to cut off a forehead or a limb in a shot.

Sorry if the original question was phrased a bit strangely
